Yes - Before approval to use ESM facilities, all new and existing ML Schedules accounts must upload and maintain the following documents:
1) A signed copy of ESM’s Acknowledgment - Automated External Defibrillator (AED) Link to printable form
- Required for all groups requesting outdoor facility use (Must use your own AED).
- Required for groups using their own AED indoors.
2) A signed copy of ESM’s Agreement for Use of Automated External Defibrillator (link to printable form)
- Required for indoor use if using an ESM-provided AED.
3) AED Certification Verification: At least one individual with a valid AED training certification must be present during each facility use. Groups must submit proof of AED certification for all individuals designated to oversee or operate the AED.
4) An AED Implementation Plan
- Required for both indoor and outdoor facility requests.
Each organization must review and update all documents annually. Organizations using their own AED are responsible for maintaining the device, ensuring it is fully functional per the manufacturer’s specifications and keeping up with training and supplies.
